Community Care Everywhere

Community Care Everywhere (CCE) is Community Health Network’s newest direct-to-employer primary care service offering. This program brings together Community’s comprehensive services including Community Virtual Care and Community Clinic at Walgreens to provide convenient healthcare access to local employer workforces as both a virtual option and/or clinical setting.

The integrated model of both virtual and in-person care enables an employer to address a spectrum of personal healthcare needs of employees and dependents. Community Care Everywhere allows your company to manage current and future health care costs in a manner that takes full advantage of the evolution-taking place in the healthcare environment.

Developed as a direct response to local employer’s rising health costs, CCE positions prospective clients as an “employer of choice” while delivering on gains in productivity, reduced absenteeism and employee retention.

The Community Care Everywhere program is structured for employers as a fixed, subscription-based multi-level PE/PM model. It offers unlimited employee utilization while providing employers valuable program insights so they can understand the value that is provided in terms of health cost avoidance and general wellness opportunities.

Benefits to Employers

Community Care Everywhere (CCE) is a “direct to employer” service:

  • Subscription/membership-based service, paid by the employer – not an insurance plan
    • 12-month contract required
    • Monthly PE/PM invoicing to the employer
  • Two tiers of membership
    • Employee only
    • Employee + Family
  • All services provided via Community Virtual Care and Community Clinics at Walgreens are covered under the membership fee
    • No additional fees for testing or other services provided on location at the Community Clinic at Walgreens
    • Referrals to elevated level of care and/or services are the responsibility of the member and is not included in the CCE program scope of services
  • No limit on the number of visits annually
  • Prescriptions will be sent to members pharmacy of choice
    • Members are responsible for payment of prescriptions
  • Online scheduling
  • Access to care 7 days per week

Services

Community Virtual Care

Community Virtual Care connects your employees and their families with board-certified providers to receive care for minor illnesses and injuries from the comfort of home. Visits are completed over the phone or through a secure video visit. Community Virtual Care is integrated with Community’s EMR and patients will have access to their after visit summary through MyChart.

Community Virtual Care is available daily from 7 am ­– 11 pm. The patient must be located in Indiana to access virtual care.

Virtual care may be used for the following medical conditions. Virtual care providers can also help:

  • Prescribe medications
  • Refill medications*
  • Order/schedule COVID-19 testing

*Please Note: We cannot prescribe medications for psychiatric needs, nor refill prescriptions for controlled substances.

Community Clinic at Walgreens

Community Clinic at Walgreens offers easy access to medical care when your employees or their families are sick or hurt and need help quickly. Community Clinic at Walgreens offers in-person care for minor illnesses and injuries, common vaccinations, and management and treatment of chronic diseases such as hypertension, diabetes, high cholesterol, and asthma.

Community Clinic at Walgreens is available weekdays from 8:30 am – 7 pm and weekends from 8:30 am – 5 pm. There are multiple, convenient locations in Central Indiana.
